Playing the decider at Kingston for the second year running, the unbeaten Guilford won 12.12 (84) to 8.10 (58) with Max Lamb kicking three, but the hosts didn't have it all their own way.
Scotch's Harry Bayles kicked the game's first from outside 50 and followed up with his side's next three as the visitors trailed by just seven points at half time.
Lawson Archer's first and three second-half goals to Will Dakin gave Scotch Oakburn fans plenty to cheer about, but a string of unanswered goals in the third term ultimately proved the difference.
Earlier in the day, St Patrick's College had just two goalkickers, but ran away easy winners in the second division grand final.
Dylan Farquhar (seven goals) and Cody Plummer (three) led the way in the 10.12 (72) to 5.7 (37) victory over Hutchins, with Fletcher Harper and James Leake also important.
Robert Zeeman was Hutchins' sole multiple goalkicker with three, while Harry Jones and Tom Griffiths also hit the scoreboard.
