The man was a passenger in a Kia Rio which was hit head-on by a Ford Laser at Lower Turners Marsh in the afternoon on Christmas Day.
He was flown by helicopter to the Royal Hobart Hospital that night, but died of his injuries on Saturday.
An 18-year-old male driver of the Kia, from Waverley, and a 34-year-old female driver of the Ford, from Mount Direction, avoided serious injury.
Tasmania Police said the driver of the Ford had veered onto the wrong side of the road, “crossing double white lines”.
On Boxing Day, a driver performed a number of burnouts on the crash scene after police had carried out a crash survey.
The fatality takes the Christmas road toll to three, and 32 for 2019.
