Dubbed the Devil of a Swim, the event has grown into an East Coast attraction.
This year's spectacle will feature a five kilometre swim around Diamond Island, along with one and two kilometre events.
Athletes 14 years old and over can compete in the five kilometre event, while the other two events are open to all ages.
Bicheno Surf Life Saving Club president Michael Symons said people keep coming back for the challenge.
"It's different swimming in deeper water and people like that - it's a challenge," he said.
"The water will be about 16 degrees. We've even got one guy coming that classes himself as an Arctic swimmer, so he has opted not to use a wetsuit."
Mr Symons said the event had a unique atmosphere and it had gained a reputation as one of the most spectator-friendly sporting events in the state.
"We've worked to publicise the event on social media, so we're expecting it to be almost twice as big as it was last year.
"We hope to get about 30 or 40 competitors in the five kilometre event."
The events will take place on Sunday, May 3.
Registration for the five kilometre event is $25, while the one and two kilometre swims are $20.
The Bicheno Surf Life Saving Club will be taking registrations until 8am, with the first race to start at 8.30am.
The Bicheno Surf Life Saving Club will also host a trial swim afternoon on Saturday, May 2, at 3pm.
Swimmers can also register at the swim on Saturday afternoon.
