Emergency services were called to the crash, under the Westbury Road overpass, about 8.15am on Tuesday.
Tasmania Police said a taxi heading north crashed into the rear of another vehicle, pushing it into a third vehicle.
The victim and her carer were passengers in the taxi, which was also struck from behind.
The woman was transported to the Launceston General Hospital.
She died at the hospital several hours after the crash.
Crash investigators were notified and were preparing a report for the coroner.
Tasmania Police Acting Inspector Nick Clark said all motorists need to drive to the conditions and allow sufficient distance between vehicles to allow time to safely stop in case of an emergency situation.
Anyone with information or dash cam footage of the crash should contact Crime Stoppers on 1800 333 000.
