The crash occurred on Gordan River Road at Glenora about 1.45am.
Police say the crash occurred between two Toyota Hilux's - one travelling east, and the other west.
Sadly, the male driver of the Hilux travelling west died at the scene as a result of his injuries.
The 18-year-old male driver and 18-year-old female front passenger of the second vehicle were taken to the Royal Hobart Hospital in a serious, but stable condition.
Crash investigators and Forensic Services remained at the scene on Sunday morning.
Police say preliminary investigations suggest the second vehicle may have crossed to the incorrect side of the road.
A report will be prepared for the coroner.
The latest crash brings the state's road death toll for the year to 21.
