A proposed extension for Lilydale Family Health Care would see the clinic's existing ground floor extended to include a treatment room, nurse room, two additional consult rooms and a playroom.
An upper floor would also be added, consisting of a staff room and additional amenities.
Located at 1977 Main Road, the former Lilydale GP Clinic closed in December 2017.
It was re-opened as Lilydale Family Health Care in January 2018, by the owners of the town's pharmacy, which is located next door.
The proposed plan, put forward by Natural Energy Designs, would allow for 15 car spaces including two disabled spaces.
The pharmacy's existing four car spaces would remain on site.
According to the plans, all consult walls would be fitted with soundproof insulation and soundcheck plastering.
The roof would also be designed to hold solar panels.
The application is open for comment until June 18.
