The AFL website on Monday said that the club had “settled’’ on Fagan as the man to replace to Justin Leppitsch.
Fagan presented to the Lions’ coaching selection panel last week after being first interviewed by chief executive Greg Swann and list manager Peter Schwab a week earlier.
Fagan played 275 games for Hobart, Sandy Bay and Devonport before entering coaching.
The former Tassie Mariners boss was Melbourne’s reserves coach and then an assistant coach, before becoming the club’s football operations manager.
Fagan has been at Hawthorn since 2008 as coaching and development manager before becoming general manager of football operations under Alastair Clarkson.
Fagan joins Rodney Eade (Gold Coast) and Brendon Bolton (Carlton) as Tasmanian head coaches in the AFL.
