In 2013 Tasmania received Commonwealth funding to improve access to palliative care across the state.
The Better Access to Palliative Care (BAPC) program is a three year project funded by the Commonwealth Government (through the Tasmanian Health Assistance Package).
The objective of the BAPC project is to increase Tasmania’s capacity to provide access to community based care for people with life limiting illness, nearing the end of their life.
We have now been funded to deliver a further year of service, meaning packages of care through hospice@HOME (h@H) are now available to people until the end of June 2017.
hospice@HOME operates through The District Nurses, a not for profit organisation, which has been delivering care to Tasmanian communities since 1896. hospice@HOME focuses on how we, as a community, can work together to support people who choose to die in their own home.

The program operates state wide and is available to any resident of Tasmania.
h@H coordinates ‘wrap around’ packages that aim to work with current service providers to support them, their families, patients and carers to fulfil end of life wishes and remain at home within their community.
h@H is currently working with 50 community service provider organisations statewide to deliver packages of care within Tasmania, and to date h@H has received 1600 referrals for care.
h@H packages are developed in a manner that is fully inclusive of the client, their family and carers and are tailored to ensure that Tasmanians who would prefer to die at home are able to do so, that their priorities for care are known and they are appropriately communicated.
The h@H packages contract a range of services which include personal care, nursing, respite, equipment, domestic support and after hours telephone advice and care.
Wherever possible, existing service providers involved in the client's care will be used to deliver these additional services.
