Tasmania Police said a vehicle participating in Targa Tasmania crashed over an embankment on Olivers Road during a closed section of the event.
Police and emergency services were called to the scene, however, the man died.
The female front seat passenger received non-life-threatening injuries.
"Our thoughts are with the man's family and his loved ones at this difficult time," Inspector Darren Hopkins said.
"Police do not have permission from the man's family to release his name at this time."
The road remains closed while a crash investigation is carried out.
A report will be prepared for the Coroner.
Wednesday's crash comes following three deaths in last year's edition of the race.
As two separate crashes claimed the lives of Shane Navin, Leigh Mundy and Dennis Neagle, Motorsport Australia handed down 23 recommendations to the event - with 17 of them falling within Targa's remit.
All but two of them are in place for Targa Tasmania while the others - involving car set-up and licencing - are works in progress.
Targa Australia chief executive Mark Perry gave his condolences to his family.
"Hand on heart, we can say that [we've done everything we can safety-wise]," Perry said.
"As I've been talking about for months now and since the tribunal released its report, we've worked tirelessly to implement everything that the experts, ourselves and the investigators found from last year.
"We are seriously at a loss given all the changes and until we get the full picture of what's actually happened, we can't rule out a medical episode, we can't rule out anything really at this point in time."
