Premier in 1967, 1976, 1977 and 1978, Pioneer-Gladstone brought together tin mine employees and workers from Rushy Lagoon, Icena and Cape Portland before winding up 41 years ago.
Saturday's 2pm clash will see East Coast don Pioneer-Gladstone's traditional guernsey in a heavyweight clash with 2019 premier Lilydale.
Swans coach Gene Savage said each of the four Pyengana heritage games so far had been days to remember.
"It's an exciting day to reunite past clubs in the North-East and to have Lilydale be the benchmark in the competition to come down and challenge us on what is a big day for our club is really exciting from all angles," Savage said.

"Marcus Haley who has come back to our club has a significant connection with Pyengana, his grandfather was an ex-player and we're going to make Marcus co-captain on the day to create some excitement around his family and pay tribute to his family and the region.
"I never forget playing there myself - it's quite an experience for the players and we're really looking forward to it.
"The whole club gets excited for this one."
The day will start with a 10.30am Auskick fixture between East Coast and Scottsdale before the reserves take to the field at 12 noon.
A host of former Pioneer-Gladstone players are set to attend the fixture with the senior heritage guernseys to be auctioned off after the game.
