Emergency services were called to the crash on Boyer Road at Dromedary on Friday evening.
A red Ford Fairmont driving towards Bridgewater and a purple Holden Commodore heading to New Norfolk collided after the Ford failed to navigate a right-hand bend and crossed onto the incorrect side of the road, police say.
The 38-year-old female driver of the Commodore was taken to the Royal Hobart Hospital in a serious condition.
Her daughter and two grandchildren, also in the car, were also taken to hospital for observation, along with the 19-year-old driver, and two passengers in the Ford.
