The Hawks knocked off Cavaliers in the preliminary final last year en route to breaking a 23-year title drought at the expense of Arrows.
This time the two sides will meet in the decider after accounting for Arrows in consecutive weeks.
Cavaliers boast a 2-1 record over their rivals this season after a 58-57 thriller in round six and a 66-55 result in round 10, but the Hawks won the most recent clash 67-57.
