Tasmania Police said the man was driving a white Mitsubishi sedan on the Bass Highway and stopped so he could turn right onto Blackabys Road.
"At this time, a Kenworth prime mover and trailer travelling east on the Bass Highway impacted with the rear of the sedan," police said.
The collision threw both vehicles off the highway and into a paddock near the Boat Harbour Beach turnoff.
Police responded to the crash at around 9.20am and the 60-year-old was transported to the North West Regional Hospital by Ambulance Tasmania.
The man was then sent to the Royal Hobart Hospital on Monday afternoon and remained in a critical condition as of 4pm.
Police said the 47-year-old driver of the truck was also conveyed to the North West Regional Hospital "as a precaution".
"The crash is being investigated by Western Crash Investigation Services," police said.
"Police are asking for anyone who saw either vehicle prior to or at the time of the crash to contact Senior Constable Adam Lloyd on 6477 7276."
