Trailing by 32 points at half-time and with only three goals to their name, the Hawks swung things around with a five-goal third term led by 42-disposal winner Jaeger O'Meara and their small forward procession of Liam Shiels, Luke Breust (three goals each) and Chad Wingard.
With the lead changing hands four times in the last quarter, the Hawks had opportunities to finish the game but gave Blues a sniff at glory before the siren sounded prior to a Harry McKay mark.
Chasing their first consecutive wins since round 13, 2017, Carlton looked the goods in the first term as they dominated possession and were boosted by early goals from power forward duo McKay and Mitch McGovern.
The Blues tall-timber was proving too be too much for Hawthorn as Matthew Kreuzer launched a bomb from outside 50 and McKay and McGovern added to their already impressive tally to have the underdogs leading by four goals.
The second quarter provided less scoring opportunites as the Hawks attempted to clamp down on the Blues pressure but a Jack Silvagni goal that Cristiano Ronaldo would have been proud of put the Hawks further on the back foot.
O'Meara slightly distinguished the pressure with clean set shot towards the end of the first-half but the Blues sat firmly, kicking eight goals in the first half to be in front at the main break by 31 points.
Hawks quelled the Blues momentum with a goal in the opening three minutes of the third as Isaac Smith combined with former Demon and Giant Tom Scully.
Shiels, Breust and Wingard then stamped their authority on the match as the Hawks clawed their way back into it with a Mitch McGovern point after the three-quarter time siren ensuring a five-point Blues lead and a barn-burner of a final-term contest.
After the Blues held the lead for slightly less than 100 minutes, Wingard gave the Hawks their first taste in front in the second minute of the final term with a goal-square boot sailing into the neighbouring Invermay Park.
Lochie O'Brien wrestled back the lead with an almost identical goal with Ed Curnow following it up by running onto a dribbling ball to extend Carlton's lead to eight points.
James Cousins, Shiels, Mitch Lewis and Breust kicked majors in the last to all but secure the Hawks' victory but Sam Petrevski-Seton and O'Brien had other ideas, kicking a goal apiece to almost claw the visitors back as the siren sounded before a possible Blues' revival.
