Zachary James McDonald, 26, has been charged with driving without a licence, leaving the scene of a crash and failing to keep left of the dividing line.
The St Helens man did not enter a plea and his matters were adjourned until November 27.
He appeared before Magistrate Simon Brown in relation to a head-on collision between a Corona sedan and a Volvo at St Marys.
The crash happened on the Esk Highway, between St Marys and the pass, about 10.15am on Sunday.
It is alleged Mr McDonald fled from the crash scene with a dog and a swag
Tasmania Police Sergeant Robert Schiwy said there was a small fire in the engine of the Volvo, but a nearby resident put it out prior to police arriving.
A 34-year-old St Marys woman had to be cut out of the Corona.
The mother sustained serious but non-life-threatening injuries.
The woman was flown by air ambulance to the Royal Hobart Hospital with multiple fractures in her legs and arms.
Her eight-year-old daughter was in the rear of the car, and Sergeant Schiwy said she was in a good child restraint.
Sergeant Schiwy said the girl was taken to the Hobart hospital by road as a precautionary measure.
Volunteer ambulance officers, the State Emergency Service and firefighters attended the scene with police.
Investigations into the crash are ongoing.
