Last year, the initiative raised $700 in two weeks and provided gifts to 62 residents.
This year, the group hopes to raise more than $2000 through personal and corporate donations, so all the home's residents can receive a special gift.
The initiative was inspired by a similar program in America.
Adopt A Grandparent co-founder Danielle Seadon said the program was as much about the emotional interaction as the exchange of gifts.
"I used to work in the home here, so I know how lonely it can be at Christmas for some people when they don't get visitors," Ms Seadon said.
"It was wonderful last year, I know a lot of the residents were really excited about it, and it was really nice," she said.
Presbyterian Care Tasmania Care services manager Peter Francis said the initiative brightened the lives of the facility's residents.
"Christmas time evokes many memories for our residents and we are grateful that initiatives such as the Adopt a Grandparent Initiative are able to support us in providing our residents with meaningful interactions at this special time of the year," Mr Francis said.
