A 25-year-old Leslie Vale woman is in a serious condition in the Royal Hobart Hospital after the smash, which occurred about 9.30am at the Midway Point Causeway.
Her grey BMW X5 veered across the intermittent white line onto the wrong side of the road, where it collided head-on with a white Isuzu truck.
The BMW then hit a white Ford Ranger ute that was driving behind the truck.
The 44-year-old male truck driver, from Taroona, and his 51-year-old male passenger, as well as the 53-year-old male Orielton ute driver, were uninjured.
The woman's 34-year-old male passenger has also been taken to the RHH.
Tasmania Police investigator Constable Tony Gough, of Sorell Police, would like to hear from any members of the public who may have seen the crash or any of the vehicles beforehand.
