A Search and Rescue crew was notified about 7.30pm that two sailors on-board the yacht Mission Performance had been injured after a strong wind gust off Cape Raoul hit their craft.
The sailors were a man from the United Kingdom and a Sydney woman.
Their yacht, which had other crew members onboard, was bound for Brisbane after it left Hobart at 5pm.
The police rescue helicopter and police vessel Relentless were activated.
The injured sailors were transferred from their yacht to the police vessel and then taken to Nubeena.
They are now in the Royal Hobart Hospital in a stable condition.
The man sustained head and suspected spinal injuries, while the woman sustained suspected broken ribs.
